鄭沛軒 0f0f1de913 fix: 修復詞卡頁面新增詞卡功能模態框顯示問題 
- 修復setShowForm未定義錯誤，添加缺失的狀態管理
- 解決模態框z-index被遮擋問題，將模態框移至最外層
- 使用內聯樣式替代CSS類名，避免樣式衝突
- 優化模態框架構：狀態提升到父組件，確保正確顯示
- 新增詞卡功能現已完全正常運作
