refactor: 大規模清理 Services 死代碼，優化後端架構
- 移除 12個完全未使用的服務文件 (-39%)
- 刪除 3個冗余資料夾 (AI/, Infrastructure/, Domain/)
- 清理 Extensions 中的死代碼服務註冊
- 移除重複實現 (GeminiAIProvider vs GeminiService)
- 移除過度設計的抽象層 (IAIProvider, IAIProviderManager)
- 簡化服務架構，從 31個文件減少到 19個文件

- HealthCheckService, CacheCleanupService, CEFRLevelService
- AnalysisCacheService, CEFRMappingService
- 整個 AI/ 資料夾 (重複實現)
